home based care
Mildmay's model of home-based care (HBC) is about capacity-building, mobilising communities and strengthening systems. It bridges the gap between mainstream health and community settings through the provision of primary nursing and multidisciplinary care.
The model we are using in Kenya has been developed in partnership with the Ministry of Health. It was successfully rolled out in Nyanza Province and in November 2006 was upgraded by the Kenyan government as the national model.
The Tanzanian government also recognises home-based care (HBC) as a critical tool in responding to the challenge of HIV+AIDS. Mildmay is reinforcing this approach in Kilimanjaro as a cost-effective way forward, reducing pressure on an overstretched and under-resourced healthcare system. Here, two patient support centres have also been opened to bridge the gap between treatment centres and HBC. Read more
As part of our HBC ethos, we support an enhanced role for the carer in assisting with nutrition, diagnosis of opportunistic infections, ARVs dosage, adherence and onward referral.
Our continued presence is having a major impact on local ability to address HIV+AIDS and relieve suffering.
